Comments:
A monitoring inspection was 4 children present, ranging in ages from 3 years to 5 years, with the provider. Children were observed while they were transitioning to go play outside and while they were playing in the backyard. Handwashing was also observed.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. There were no medications on site today. A total of 3 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-100-A
Description: Based on record review, one child record did not contain a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 (start date 09/06/2022) did not contain the results of a physical examination.

Plan of Correction: The provider shall contact the parents of Child #1 and request the results of the physical examination.

Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the fdh did not keep a complete written record of children in attendance each day.
Evidence:
1) On the date of inspection, there were 4 children in attendance. The attendance record for the day had only the name of 1 child
documented as present for the day.

Plan of Correction: Corrected during inspection. The provider added to the attendance list the names of all the children present.

Standard #: 8VAC20-800-60-B
Description: Based on record review, one child record did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the first and last dates of attendance.

Plan of Correction: The provider shall complete the missing information in the record.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not always installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
1) On the date of inspection. the inspector observed one uncovered outlet in the classroom area and three uncovered outlets in the lobby area.

Plan of Correction: The provider shall install protective covers over the unused electrical outlets.

Standard #: 8VAC20-800-320-G
Description: Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) The most recent fireplace inspection was dated 12/14/2019.

Plan of Correction: Within 15 calendar days, the inspector will follow up with the provider to obtain an update on the fireplace inspection.
