All caregivers are require to obtain 16 hours of training annually in addition to having current First Aid and CPR.

Comments:
An unannounced renewal inspection was conducted on October 13th, 2022. There were 8 children present with 2 caregiver's supervising. The home, children's records, caregiver records, household member records, attendance logs, evacuation logs, the first aid kit and menu's were reviewed. The children were observed playing, hand washing and eating breakfast that consisted of pancakes, bananas and milk. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #2, #3, #4 and #5 record did not contain documentation of a physical examination by the 30th day of attendance. Child #2' first day of attendance - 8/15/2022- Child #3's first day of attendance - 9/6/2022 - Child #4's first day of attendance - 8/23/2022 and Child #5's first day of attendance - 8/3/2021.

Plan of Correction: I will get the information from the parent and place in child file.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2's record does not contain documentation of the telephone number of each custodial parent's place of employment.
Child #1 and child #5's records do not contain documentation of the review of the child's emergency contact information.

Plan of Correction: I will get the information from the parent and put in child file.

Standard #: 8VAC20-800-80-A
Description: Based on a review of children's records, the provider failed to obtain verification of identity and age by the child's seventh day of attendance at the family day home.

Evidence:
Child # 3, #4, and # 5's records do not contain verification of identity and age.

Plan of Correction: I will get the information from parents and put in child file.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized by the first day of attendance, according to the requirements of the Code of Virginia.

Evidence:
Child # 2, #3, and #4's record do not contain documentation of immunizations by the first day of attendance. Child #2's first day of attendance-8/15/22 - Child #3's first day of attendance 9/6/2022 and Child #4's first day of attendance 8/23/2022.

Plan of Correction: I will get the information from the parent and place in the child file.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, the provider failed to ensure that all caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to the job responsibilities.

Evidence:
Caregiver #1 and Caregiver #2's records do not contain documentation of annual training hours.

Plan of Correction: We will complete the annual training hours required.
