Comments:

The Licensing Inspector initiated and completed a monitoring inspection on 10/12/2022. The inspector was on site from 9:45m until 11:20am. There were three children under the care of one provider. The ages of the children ranged from two years old to three years old. The children were observed during group time and snack time. Three children?s recordone staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program Nicole Scott Office of Child Care Health and Safety Division of Early Childhood Care and Education Phone# 804.588.2373 nicole.scott@doe.virginia.gov

Standard #:	22.1-289.036-A
Description:	Based on record review, the provider failed to obtain an updated search of the central registry check after 5 years. Evidence: During the inspection on 10/12/2022, the provider failed to provide an updated central registry results check. The last central registry check was completed on 7/14/2017.
Plan of Correction:	Will send paperwork off to obtain a current Central Registry Check within a month.

Description:	Based record review, the provider failed to obtain address information for two designated persons to contact in case of an emergency if parent cannot be reached. Evidence: Child #2 was missing address for both emergency contacts during inspection on 10/12/2022.
