Comments:
A monitoring inspection was initiated on May 25, 2022 and concluded on June 7, 2022 regarding a self-reported incident. The provider reported an incident regarding allegations in the behavior guidance on May 25, 2022. Video footage was reviewed and interviews were conducted. One caregiver record, one household record and one child record were reviewed.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice. Description: Based on review of video footage and interview, the provider did not ensure that ?time-out? when used as a discipline technique does not exceed one minute for each year of the child?s age.

Evidence:

Video viewed on May 25, 2022, showed child #1 (4 years in age) being placed into the infant room for approximately 45 minutes away from his peers. During interview on June 6, 2022, the provider stated that the child was in ?time-out? because he was having a rough morning and that she was totally incapacitated to the point where she couldn?t care for him.

Description: Based on observation and interview the provider did not ensure that physical punishment, a forbidden action, was not used.

Evidence:

The following was observed on video footage provided by the provider-
1. Video footage viewed on May 25, 2022 showed household member #1 roughly handled a child in care. In the video, Household Member #1 was observed carrying Child #1 (4 years old) into the infant room and roughly forcing the child to assume a sitting position on the floor. Child #1 could be seen crying and kicking while being carried into the infant room.
2. Caregiver #2 was seen in the video footage lifting up her shirt and showing the child her belt. During interview on June 6, 2022 and June 7, 2022, the provider stated caregiver #1 had shown her belt to the child and told the child ?his mom was going the spank him with a belt when she heard how he acting?.
