Comments:
An unannounced renewal inspection was conducted today from 11:00am-12:30pm. There were 4 children directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Medications and authorization forms were reviewed. Children were observed playing freely. There was an adequate supply of books, toys, and materials for the children. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the provider did not obtain a fingerprint based background check for each staff member prior to their first day of employment.

Evidence:
Staff#1's record did not include a fingerprint record check prior to their first day of employment. The staff member's first day of employment was documented as 10/11/2022

Plan of Correction: She had 3 days of having started, I showed the inspector that I had the appointment of the fingerprints for next day, Friday October 14th.

Standard #: 8VAC20-800-750-A
Description: Based on observation and interview, topical skin products were not labeled with the child's name.

Evidence:
Two containers of diaper ointment were not labeled with the child's name.

Plan of Correction: The children's names were written on, but looked smudged from everyday use.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain a physical examination for each enrolled child within 30 days of attendance.

Evidence:
The records for Child #1 and Child #2 (DOE: 8/22/2022) did not contain physical examinations.

Plan of Correction: On the same day of the inspection, I informed the parents of that child and they sent me the physical exam.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not all contain required information.

Evidence:
1.Child #1's record was missing a second emergency contact.
2.Child #3's record was missing a second emergency contact, work phone numbers for both parents and the mother's place of employment.

Plan of Correction: The parents brought the complete information the next day.

Standard #: 8VAC20-800-170-A
Description: Based on record review, the provider did not obtain for each staff member a Report of Tuberculosis (TB) Screening form documenting the absence of tuberculosis in a communicable form prior to coming into contact with children.

Evidence:
Staff #2's (DOH: 10/11/2022) record does not contain a TB screening form documenting the absence of tuberculosis prior to coming into contact with children.

Plan of Correction: She had 3 days of having started, I showed the inspector that I had the appointment of the TB test for Tuesday, October 18th.
