Comments:
An unannounced renewal inspection was conducted today. There were 10 children (33 points) present with three caregiver's supervising. The home, children's records, staff records, household member records, bottles, cribs, menu's, evacuation logs, attendance logs, training records and emergency preparedness procedures were reviewed. Violations:
Standard #: 8VAC20-800-960-E
Description: Based on observation, the provider failed to ensure that bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
There were 3 bottles in the refrigerator that were dated, but had no name.
There were 2 containers of food that did not have a name or a date.

Plan of Correction: I will remind parents to label children's food brought from home with the name and the date. If parent's forget to label, caregiver's will label all food brought from home.

Standard #: 8VAC20-800-120-B
Description: Based on a review of caregiver records, the provider failed to obtain for assistants and substitute providers who are not the spouse, parent,
sibling, or child of the provider and are hired after June 30, 2010,
documentation that two or more references as to character and reputation as
well as competency were checked before employment.

Evidence:
Caregiver #2's record does not have documentation of two or more reference checks.

Plan of Correction: I will have reference checks completed as soon as possible.

Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each enrolled child.

Evidence:
Child #4's record does not contain documentation of names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will ask parents for the information and place in children's files.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children's records, the provider failed to ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
Child #1's record for immunizations is dated 6/23/2022, the child's first day of attendance was 6/21/2022.
Child #2's record does not contain documentation of immunizations on the first day of attendance. the child's first day of attendance was 8/23/2021.
Child #3's record for immunizations is dated 10/6/2022, the child's first day of attendance was 10/3/2022.

Plan of Correction: I will make sure that children will have proof of immunizations on the first day of attendance.

Standard #: 8VAC20-800-180-A
Description: Based on a review of staff and household members records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, n accordance with the requirements in 8VAC20-800-170, every two year form the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
Caregiver #1 does not have documentation of a current tuberculosis screening. Date of last screening 06/20/2020.
Household Member #1 does not have documentation of a current tuberculosis screening. Date of last screening 8/29/2019.

Plan of Correction: I will get my TB screening on Friday 10/21/2022 and the household member will also get the screening done on 10/21/2022.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, the provider failed to ensure that in addition o satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #1, #2 and #3's records do not contain documentation of 16 hours of training for the 2021-2022 year.

Plan of Correction: All caregiver's will have annual training completed by 12/31/2022.
