Comments:
A monitoring inspection was initiated on 10/18/2022 and concluded on 10/18/2022. There were a total of 4 children (10 points) observed with 2 caregivers present. The children were engaged in the following: toy play, manipulatives and singing their ABC's. The home, 4 child records, 2 caregiver records, evacuation drills, emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-830-A
Description: Based on a review of fire drills and provider interview, the emergency evacuation procedures have not been practiced monthly with all caregivers and children in care.
Evidence: The emergency evacuation drill was missing the month of September 2022. The provider stated that a drill was not conducted.

Plan of Correction: Fire drills will be conducted monthly and documented on the form.

Standard #: 8VAC20-800-50-A
Description: Based on a review of attendance records and provider interview, the provider has not kept a written record of children in attendance each day.
Evidence: There was no current attendance record for the 4 children in care. The Provider stated the last attendance record taken was in June 2022.

Plan of Correction: The provider will maintain daily attendance.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children records, each child's record does not contain name, address, telephone number of two persons to contact in case of an emergency and the parent's signed acknowledgement of the Parent Information form.
Evidence: Child #3 and child #4, first day of attendance 8/12/2022 does not have a second name, address and telephone number to contact in case of an emergency.
Child # 4, first day of attendance 8/12/2022 does not have the parent's signed acknowledgement of the Parent's Information form.

Plan of Correction: Parents will complete the missing documents and the provider will file them in the child's record.

Standard #: 8VAC20-800-70-B
Description: Based on a review of children records, the provider did not obtain the parent's written acknowledgement of the receipt of whether or not there is liability insurance, the provisions of the emergency and response plan and the policies for administration of medication.
Evidence: Child #1, first day of attendance 5/9/2022 does not have written acknowledgement of the receipt of whether or not there is liability insurance, the provisions of the emergency and response plan. (Repeat)

Child #4, first day of attendance 8/12/2022 does not have written acknowledgement of receiving the policies for medication administration.

Plan of Correction: Parents will complete the missing documents and the provider will file them in the child's record.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver records, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence: Both caregivers acknowledged they had not updated their TB screenings.
Caregiver #1 TB screening was dated 9/13/2019
Caregiver #2 TB screening was dated 7/17/2019

Plan of Correction: Caregivers will update their TB screening.
