Technical Assistance:
Discussion of a crib observed on site without a manufacture date. Provider stated that the crib was purchased through an on-line retailer. The Provide will search to see if she still retained the documentation that came with this new crib.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 10/18/2022, between the hours of 10 am to 1 pm. There were 6 children present (21 points), ranging from infant to toddler-age, with 4 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 8 children records, 4 household records, and 4 staff records were reviewed.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable code. This violation was documented on the Violation Notice issued to the home.

Standard #: 22.1-289.035-B-4
Description: Based on review, 1 of 4 staff files indicated that the family day home (FDH) did not obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry (CPS check) from any state in which the individual has resided in the preceding five years before reporting to the child day center, family day home.

Evidence -
1. On the date of inspection (10/18/2022), an out-of-state CPS check was not available for Staff #3.
2. Based on documentation on file, Staff #3 indicated that they had resided in another state within the past 5 years.
3. Staff #3 was observed on site providing care to children on the date of inspection. The start date for Staff #3 was listed as 08/30/2022.

Plan of Correction: A request for the out-of-state CPS check for Staff #3 will be sent.
