A subsidy health and safety (SHSI) inspection was conducted on October 21, 2022, between 10:15am and 11:15am. There are no children currently enrolled in the FDH. The physical plant, and records were reviewed for the caregiver and the household member. The exit interview was conducted with the provider/vendor. No violations were observed.
