Comments:
An unannounced monitoring inspection was conducted from 10:00 - 10:45 am with the provider and an assistant. There were eight children, ranging from 23 months to four years of age, for a total of 17 points. Children were observed playing with duplos. Two child records, three caregiver/household member records, the emergency drill log and the daily attendance were reviewed. The provider is not currently certified in Medication Administration Training (MAT) and cannot administer medication to children in care. Standard #: 22.1-289.035-A
Description: Based on review of three caregiver/household member records, it was determined that the provider did not have a repeat central registry report for all caregiver/household members every five years as required. Evidence: The record for caregiver #1 had a central registry search report dated: 4/3/17. The record for caregiver #2 had a central registry search report dated: 4/3/17. The record for household member #1 had a central registry search report dated: 8/3/17.

Plan of Correction: Provider stated they will all obtain an updated Central Registry Search report.
