Technical Assistance:
Consultation was given on model forms, the VDOE Licensing website, qualifications for a new assistant (will need background checks, TB, first aid and CPR certification and the subsidy Pre Service 10 hour training. The 10 hours can count towards the annual 16 hours required per year the first year its taken.

Comments:
An unannounced renewal study was conducted today. Three children were present with one caregiver for five points. Observed children coloring and reading, free play and preparation for lunch. Homemade pork pupusas with cheese, beans, lettuce and tomato will be served for lunch. Reviewed trainings, 3 children's records, emergency preparation, and transportation standards. Provider participates in the food and nutrition program, subsidy and the Quality endorsements by the state (VQB5 and PDGB5). Dedicated space in the basement is set up for care of children. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on provider statement, a carbon monoxide detector is not in the home.
Evidence:
Provider stated that no carbon monoxide detector was available.

Plan of Correction: Will purchase one and install by the end of the week.

Standard #: 8VAC20-800-60-B
Description: Based on record review, 3 of 3 records were incomplete.
Evidence:
Child #1 and Child # 2 were missing the fieldtrip permission form.
Child #3 was missing an annual update. Last reviewed 4/2021.

Plan of Correction: Provider will have parent complete the missing forms.
