Comments:
A complaint investigation was initiated on September 26, 2022, from a report received by the Fairfax Licensing Office on September 22, 2022. The complaint alleged lack of supervision, an injury not reported, and not following the program's own policies. The allegations were investigated and the investigation was completed on September 30, 2022.

Complaint related: Yes
Description: Based on record review and interview, it was determined the Provider did not record in the child's record an injury or accident sustained by a child while at the family day home that requires first aid.
Evidence:
1. Provider when interviewed stated there have been injuries that required her to apply first aid and that she told the parent at the end of the day. The provider could not detail the date of the injuries but acknowledged they had occurred.
2. Provider was asked for the injury reports but she stated she didn't document the accidents. There was no written record to include details of each injury which include: date, time, circumstance of injury, caregiver present, action taken, date/time parent was notified, future action to prevent recurrence, and how parent was notified.

Plan of Correction: I will use the injury report and make sure each injury is completely documented.
