A monitoring inspection was conducted on October 25, 2022. There were 5 children present, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-780-B Description: Based on review of four children's records, one does not contain documentation of an annual parent review of the emergency contact information.
Evidence
The records for children #1 (start date 11/9/20) and #3 (start date 9/28/20) do not contain documentation that the provider reviewed with the parent the emergency contact information annually.Plan of Correction: I will have the parents review the emergency contacts, make any changes and sign and date the form. I will make sure the emergency contacts are reviewed with each parent annually.

The record for child #2 did not contain any emergency contact information, including the name, address and phone number for two designated people to contact in case of an emergency and a parent cannot be reached.Plan of Correction: I will have the parents complete the emergency contact information at pick up today. I will make sure I have all the information before a child starts.

The record for child #1 did not contain documentation that the parent was provided the information on the status of whether or not the family day home has liability insurance .Plan of Correction: I will have the parent sign it today and make sure all the records are complete before a child starts.
Standard #: 8VAC20-800-280 Description: Based on observation potentially poisonous substances, and supplies were accessible to children.

Located in lower cabinets, on counter tops, and in a bathroom (the children do not use) were cleaning agents, disinfectants, and deodorizers accessible to children.Plan of Correction: The bathroom the children never use, I will make sure the door is closed and they do not have access to it. The other items will be moved higher or put in a locked cabinet to make them inaccessible to the children.
Standard #: 8VAC20-800-320-B Description: Based on observation child-resistant protective covers were not installed on all unused electrical outlets.
Evidence
Three unused outlets that are accessible to children do not have child-resistant protective covers.Plan of Correction: I will cover the unused outlets and make sure they are always covered after using them.
Standard #: 8VAC20-800-430-C Description: Based on observation, children have access to stairs with three or more risers that do not have protective barriers.
Evidence
The stairs have more than three risers and do not have any protective barriers, making them accessible to the children.Plan of Correction: I will put a gate at the bottom of the stairs.
