Comments:
A renewal inspection was conducted on 10/27/2022 between the hours of approximately 9:15 am to 10:45 am. There were 12 children present, ranging in ages from 5 months to 3 years, with 3 staff supervising. At the time of entrance, older children were observed during circle time with the Provider: they reviewed colors and shapes, sang the ABC and the Hockey Pockey songs. After that, the provider offered puzzles to the children. Four infants were observed in their cribs.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site today. A total of 12 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-770
Description: Based on review of emergency supplies, a working portable battery-operated weather band radio, and extra batteries were not kept available to caregivers at all times.
Evidence:
1) On the date of inspection, a battery-operated weather band radio was available in the family day home, however, the batteries did not work and there were not enough additional batteries available to make it work.

Plan of Correction: The new batteries were installed and extra batteries were kept. The weather band radio is working now.

Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review in 4 out of 12 records reviewed.
Evidence:
1) The most recent review of the emergency information in the record for Child #3 was dated 07/09/2021, for Child #4 was dated 08/04/2021, for Child #5 was dated 07/07/2021, for Child #6 was dated 10/08/2021.

Plan of Correction: I will review the emergency contact information with the parents to ensure the information is correct and get their signed acknowledgement.

Standard #: 8VAC20-800-800-B
Description: Based upon review of the emergency preparedness plan, the provider has not ensured that the emergency preparedness plan is reviewed annually and the review is documented.
Evidence:
1) The most recent review of the Emergency Preparedness Plan documented by the provider was dated 10/08/2021.

Plan of Correction: The emergencvy preparedness plan was reviewed today, 10/27/2022.

Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the family day home (fdh) did not keep an accurate written record of children in attendance each day.
Evidence:
1) At the time of entrance, the inspector observed 12 children in care in the fdh. The attendance record had documented that only 11 children were in care.
2) Upon review of the attendance record and all children's records, the inspector noted that two of the children listed in the attendance list did not have records in the fdh. After inquiring the provider about it, she stated that they did not attend the fdh. The provider proceeded to update the attendance with the names of two children that were present but not included in the attendance list.

Plan of Correction: The provider corrected the attendance list during the inspection: removed the names of the two children that were not present and added the names of the three names missing.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the telephone number of each custodial parent's place of employment.

Plan of Correction: The parent updated the telephone number today.

Standard #: 8VAC20-800-540-A
Description: Based on observation, the safety strap of a high chair was not used and securely fastened, when occupied by a child.
Evidence:
1) The inspector observed a child sitting on a high chair after having finished eating breakfast. Upon inspection, the licensing inspector noted that the safety strap of the high chair was not fastened to secure the child.

Plan of Correction: I will make sure to fasten the safety strap, when children are sitting on high chair.
