Comments:
A monitoring inspection was conducted on October 26, 2022. There were twelve children present, ranging in ages from ten weeks to two years with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on review of three children's records, program failed to ensure that parents were provided in writing whether the family day home carried liability insurance.

Evidence:
1. There was not a completed liability insurance declaration form in the record for Child #1 and Child #2.
2. Provider confirmed they were not completed and stated it would be provided at pick up.

Standard #: 8VAC20-800-90-A
Description: Based on review of three children's records, program did not ensure that documentation of adequate immunization was obtained before a child may attend the family day home for one child.

Evidence:
1. The first day of attendance for Child #1, dob of 8/9/2022, was 9/26/2022. There were no immunizations on file.
2. Provider confirmed they were not provided and reached out to parent.
