Comments:
An unannounced on-site renewal inspection was conducted on 10/28/2022. There were 9 children, ages 1 year-3 years, and 1 staff member at a total of 19 points. At 9:23 AM the assistant arrived. The inspector observed the following: lunch, handwashing and nap. A total of 4 children?s records and 2 staff records, were reviewed. The inspector arrived at 9:15 AM and departed at 11 AM.

Standard #: 8VAC20-800-120-B
Description: Based on review of staff records, the licensee failed to ensure that the required two references were obtained prior to starting the position.

Evidence:
Staff # 1 started 10/28/2022 and did not have the two required references.

Plan of Correction: The References will be completed today.

Standard #: 8VAC20-800-170-A
Description: Based on review of staff records, the licensee failed to ensure that the TB screening or test was completed prior to starting the position.

Evidence:
1. The Staff # 1 started 10/28/2022 and did not have the required TB screening or test prior to coming into contact with children.

Plan of Correction: The TB screening will be completed and will be completed prior to hire in the future.

Standard #: 8VAC20-800-570-A
Description: Based on observation and record review, the licensee failed to ensure that a caregiver does not exceed 16 points.

Evidence:

1. The licensee had eight children in care at a total of 19 points from the time of the inspectors at 9:15 AM to 9:23 AM when the assistant arrived.

Plan of Correction: The normal assistant had a family emergency and was not able to make it on the day of the inspection. The licensee called in the new person and she arrived as quick as possible.
