Comments:
An unannounced monitoring inspection was conducted on 11/2/2022 from 10:30am-12:15pm. There were 8 children (21 points) supervised by 2 direct caregivers. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely and playing outside. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review and interview, the Provider failed to obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years

Evidence:

1. There was no documentation of an out-of-state child abuse and neglect search request by the end of the 30th day of employment for Caregiver #2.

2. There was no documentation of an out-of-state criminal history name check prior to the 1st day of employment for Caregiver #2.
