Comments:
An unannounced monitoring inspection was conducted on November 1, 2022. There were 8 children (27 points) with 2 caregiver's supervising. The children were observed finishing breakfast, singing songs in English and Spanish and playing with floor toys. The home, attendance logs, evacuation logs, cribs, bottles, children's records, caregiver's records, household member records, first aid kits and menu's were reviewed. Violations:
Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child # 1, #3 and #5's records do not contain documentation of the telephone number of each custodial parent's place of employment.

Plan of Correction: I will get the information from the parents and put it in the children's file.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the provider failed to ensure that caregivers shall wash their hands with liquid soap and warm running water after diapering a child with toileting.

Evidence:
After changing three children's diapers, Caregiver #2 changed gloves, but did not wash her hands with soap and running water after each diaper change.

Plan of Correction: I will retrain caregiver's on handwashing procedures.

Standard #: 8VAC20-800-680-B
Description: Based on observation, the provider failed to ensure that caregivers shall make certain that children's hands are washed with liquid soap and warm running water after toileting or diapering.

Evidence:
Based on observation, Caregiver #2 failed to wash children's hands with soap and running water after toileting/diapering.

Plan of Correction: I will retrain caregiver's on handwashing procedures.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be cleaned and sanitized after each use.

Evidence:
Caregiver #2 did not clean and sanitize the diaper surface was not cleaned and sanitized after each use.

Plan of Correction: I will retrain caregiver's on cleaning and sanitizing diapering surface.
