An unannounced renewal inspection was conducted on 11/01/2022. There were 5 children, ages 3months ? 3 years, totaling 15 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records, 1 caregiver?s records and 1 household member?s records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 10:20am and departed at 11:43am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.036-A Description: Based on the review of one caregiver?s record, the provider failed to ensure that every adult living in a family day home shall undergo all background checks in accordance with the Code of Virginia prior to the issuance of a license and every five years thereafter.
Evidence: The most recent sworn statement in the record for Caregiver 1 was signed and dated 12/17/2015. There was not a signed updated sworn statement completed in 2020.Plan of Correction: Provider will complete an updated sworn statement.
