Discussion that the 2 Fingerprint checks on file are set to expire in February 2023.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 11/04/2022, between the hours of 10 am to 11:30 am. There were 4 children present (15 points), ranging from infant to toddler-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, and 1 staff record, and 1 household record was reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 4 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (11/04/2022), the following children files was missing their policy number for the listed policy name: Child #2, and Child #4.
2. The start date for Child #2 was listed as 10/22/2022, and the start date for Child #4 was listed as 07/01/2022.

Plan of Correction: For those children not leaving this month, all missing information will be obtained.

Standard #: 8VAC20-800-90-A
Description: Based on review, 1 of 4 children files indicated that the Family Day Home (FDH) did not obtain documentation that each child has been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) prior to their start in the child care program.

Evidence -
1. On the date of inspection (11/04/2022), documentation of the immunization page for Child #3 was not available for review.
2. The start date for Child #3 was listed as 10/17/2022. Child #3 was in attendance of the date of inspection.

Plan of Correction: For those children not leaving this month, all missing information will be obtained.
