Comments:

An unannounced monitoring inspection was initiated and completed on November 7th, 2022. There were five children (14 points) present with 2 caregivers. The home, children's. records, caregiver's records, household member records, menu's, first aid kit, attendance logs and evacuation logs were reviewed. There were no medications on site. The children were observed playing with floor toys, reviewing the days of the week and singing songs in English and Spanish. Areas of non-compliance were cited on the violation notice. Standard #:	8VAC20-800-180-A
Description:	Based on a review of household member records, the provider failed to obtain for each adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening . Evidence: Household Member #1's record did not contain documentation of a current Tuberculosis screening. Last screening 9/29/2020.
Plan of Correction:	My household member will get his TB screening next week.
Standard #:	8VAC20-800-680-A
Description:	Based on observation, the provider failed to ensure that all caregivers wash their hands with liquid soap and warm running water after diapering a child. Evidence: Caregiver #2 changed 2 children's diapers, but did not wash her hands after each diaper change.
Plan of Correction:	I will make sure we wash our hands after changing diapers.

Description:	Based on observation, the caregivers failed to ensure that children's hands are washed with liquid soap and warm running water after a diaper change. Evidence: Caregiver #2 did not wash children's hands after a diaper change.
Plan of Correction:	I will make sure all children's hands are washed after diapering.
Standard #:	8VAC20-800-690-D
Description:	Based on observation of the diapering area it was determined that the diaper surface was not nonabsorbent. Based on observation, the caregiver's failed to ensure that the diapering surface was cleaned and sanitized after each use. Evidence: The changing pad had several holes that rendered it nonabsorbent. Caregiver's did not clean or sanitize diapering surface after each used.
Plan of Correction:	I will purchase a new changing pad and I will make sure it is cleaned and sanitized after every use.
