Comments:

An unannounced monitoring inspection was conducted on 11/04/2022 from 9:45am to 10:45am. There were 2 children ages 2 years old to age 3 years old (4 points) with the Provider. The programming area, outdoor playground area, menus, Provider and 2 Household member records, 2 children?s records, attendance, emergency drills, and emergency supplies were reviewed. There were no medications on site. Children were observed participating in dramatic play. Handwashing procedures were also observed. The Provider was current in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 11/14/2022. Description:	Based on review of 3 household member records, the provider did not obtain a central registry background check report for each Household member every five years from the date of the last check. Evidence: The record for Household member #2 and Household member #3 did not contain documentation that a central registry background check report had been completed every five years from the date of the last check. The last central registry check on file for Household member #2 was completed 5/24/2016 and the last central registry check on file for Household member #3 was completed on 6/13/2017.
Plan of Correction:	New forms for registry background checks went out already. Forms received 11/8/2022 at the Central Registry.
