Comments:

An unannounced monitoring inspection was conducted today from 12:50 pm-2:50 pm. There were 3 children (8 points) supervised by 1 direct caregiver. The home, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Standard #:	8VAC20-800-60-B
Description:	Based on records review, the children's records were not complete. Evidence: 1. Child # 1 and Child # 3's records were missing the health insurance policy numbers. 2. Child # 3 was missing the name of the child?s medical insurance. 3. Child #1 and Child # 3?s records were missing the name, address, and telephone number of one designated person to contact in case of an emergency if the parent cannot be reached.
Plan of Correction:	Rec'd policy numbers for #1 and #3. Rec'd child's medical insurance. Rec'd emergency contact for children.
Standard #:	8VAC20-800-70-A
Description:	Based on record review, the Provider did not provide in writing the following information to parents before the child?s first day of attendance. Evidence: There was no documentation of the medication administration policy for Child # 1 (date of attendance: 11/1/2022), Child # 2 (date of attendance:: 11/7/2022), and Child # 3 (date of attendance: 09/1/2022).
Plan of Correction:	Documentation has been placed in their file and on bulletin board for parents to see.
Standard #:	8VAC20-800-80-A
Description:	Based on record review, the Provider did not obtain verification of the identity and age of the child within seven business days of the child?s first day of attendance at the family day home. Evidence: There was no documentation of verification of identity and age for Child #3 within seven business days of the child?s first day of attendance at the family day home. Child #3's first day of attendance was 9/1/2022.
Plan of Correction:	I will have the parent fill out the form but I do have his birth certificate.
