Discussion of background checks that are set to expire in 2023. These include those CPS checks, FPC, and SDS that are dated 2018.
Discussion of submitted FDH Renewal Application and attached documentation for the LLC.

This renewal inspection was initiated by licensing staff and concluded on 11/15/2022, between the hours of 10 am to 12 pm. There were 4 children present (13 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 6 children records, 1 staff record, and 5 household records were reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the center.

Standard #: 8VAC20-800-180-A Description: Based on review, 2 of 5 household records indicated that the Provider did not obtain for each adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 8VAC20-800-170), every two years.
Evidence -
1. On the date of inspection (11/15/2022), the following household members had TB documentation on file that was older than 2 years: Household Member #1, and Household Member #3.
2. The dates listed for the TB documentation on file were as follows: For Household Member #1 (TB documentation was dated - 09/18/2020), and For Household Member #3 (TB documentation was dated - 09/14/2020).Plan of Correction: TB documentation will be updated for both by 02/01/2023.
