Comments:
An unannounced monitoring inspection was conducted today from 9:40am-10:35am. There were 9 children for a total of 24 points directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today and the Provider is currently MAT certified. Children were observed playing freely and eating snack. Hand washing and care of a sick child was observed. There was a large assortment of age appropriate materials for children and the home was clean and organized. The Provider was also observed facilitating a fun circle time activity and the children were singing along and enjoying a story. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-60-B
Description: Based on review of children?s records it was determined that the Provider did not obtain plan for a child with a food allergy.
Evidence:
1. Child #1 has a listed food allergy but there is no plan of action, written by the Parent, of what the Provider should do in an emergency.

Plan of Correction: I will get a plan form the parent.

Standard #: 8VAC20-800-90-C
Description: Based on record review of children?s files, it was determined the Provider did not obtain current immunization update for child #3.
Evidence:
1. Child #3 has been enrolled in home for last year and is younger than two years of age. The most recent immunization record for child #3 is from September, 2021.

Plan of Correction: I will ask parent for an updated immunization record.

Standard #: 8VAC20-800-320-G
Description: Based on record review and interview, it was determined the Provider did not ensure the wood burning fireplace received an annual inspection.
Evidence:
1. Provider was cited in April for not having fireplace inspected since March, 2021.
2. The most recent inspection document available for review was from March, 2021. The Provider said she has not had it inspected yet.

Plan of Correction: I will get them to come out in the next week.
