Comments:
An unannounced renewal inspection was conducted on 11/9/22 from 11:45am to 1:00pm with the Director and two assistants. There were nine children in care with a total of 25 points. Children were observed napping. Five child records, the provider record, two caregiver records, one household member record, and one pet record were reviewed. The provider and both assistants have current certification in CPR and First Aid. The provider has current certification in Medication Administration Training, though she reported no medications were on site for children. The first aid kit, flashlight, weather band radio, fire extinguisher, and carbon monoxide detector were observed. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 11/17/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.035-B-2
Description: Based on review of two caregiver records, the provider did not obtain the results of a national fingerprint-based criminal record check prior to employment for one caregiver. Evidence: The fingerprint results on record for Caregiver C, with an employment date of 7/30/22, were dated 9/7/22.

Plan of Correction: An appointment was made in a timely manner for caregiver C but the finger printing office did not send me the results on time, after a phone call to solve this delay I was advised to make a new appointment and start all over again. For the future I will save all the dates and paperwork regarding this matter so I can show proof and avoid future inconvenience.

Standard #: 8VAC20-800-180-A
Description: Based on review of caregiver and household member records, the provider did not obtain for each caregiver and adult household member a current Tuberculosis (TB) Screening every two years from the date of the first screening. Evidence: The TB screening on record for Household Member A was dated 9/11/2020. The TB screening on record for Caregiver B was dated 10/8/2020.

Plan of Correction: Documentation for both TB test swill be provided next week.
