Comments:
An unannounced renewal study was conducted from 1:00 pm - 3:00 pm. There were five children in care with the provider for a total of 13 points. The children were observed resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records two staff records and three household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-120-B
Description: Based on review of one assistant caregiver records it was determined that not all records were complete. Evidence: The record for caregiver A did not contain the following: Name, address, verification of age, job title, date of employment, emergency contact, and two references.

Plan of Correction: The provider will request the missing information from the caregiver.
