Technical Assistance:
Today we discussed the following topics:
Antibacterial Soap (use with direct supervision)
Use of high chairs and other equipment that children cannot exit on their own - only when being used for their intended use and remove from equipment after no more than 30 minutes with at least one hour between different equipment. - importance of free movement and supervised tummy time for development.
Posting inspection notices so that parents can comfortable view any violation notices.
Carefully reading and following manufacturer's instructions for equipment.
Sleeping spaces for infants - cribs only - moved to their individual sleeping space when they fall asleep elsewhere.

Comments:
A renewal inspection was initiated and concluded on 11/17/2022. At the time of the inspector's arrival, there were 7 children present, ranging in ages from 4 months old to 2 years old, with the licensee and 2 household adults supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 caregiver / household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on a review of records, the licensee was unable to demonstrate that a physical examination by or under the direction of a physician was obtained prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
The record for child #5 whose enrollment date was documented as 1/3/2022, did not contain the results of a physical examination.
The licensee confirmed that she did not have results of a physical examination for child #5.

Plan of Correction: The licensee has spoken with the parents of child #5 and an appointment for a physical has already been secured.
The results of the physical examination will be placed child #5's record upon receipt.

Standard #: 8VAC20-800-50-A
Description: Based on observation, the licensee did not ensure that a written record of attendance is kept each day.
Evidence:
There were 7 children present at the time of inspection on 11/17/2022.
The attendance record for the family day home showed children in care on Monday, November 14, 2022.
There were no children listed as present on November 15, 16 or 17, 2022.

Plan of Correction: The licensee stated that she would ensure that children are recorded on the attendance forms daily, as they arrive and as they depart, to ensure that the attendance record is always accurate at any given moment.

Standard #: 8VAC20-800-60-B
Description: Based on a review of records, the licensee did not ensure that each child enrolled had a complete record containing all required information and documentation.
Evidence:
Five enrolled children's records were reviewed.
The records for child #1 and child #4 did not contain a name address and phone number for 2 emergency contacts to be contacted in the event of an emergency if the parents could not be reached.

Plan of Correction: The licensee stated that she has requested this information form the parents and will document the missing emergency contact information in the records for child #1 and #4 by the end of the day on Friday.

Standard #: 8VAC20-800-200-A
Description: Based on observation and interview, the provider was not able to demonstrate that assistant caregivers received orientation training by the end of their first week of assuming job responsibilities.
Evidence:
At the time of the inspector's arrival, there were 5 children downstairs as well as 2 infants upstairs in two separate rooms with the provider's husband (household member #2) and with the provider's daughter (household member #3).
The files for household member #2 and household member #3 did not contain documentation of orientation training having been conducted. Household member #3 was identified as an assistant caregiver at the last license renewal in November 2021.
When asked if either assistant had received orientation training, the provider stated that she was not aware that they were required to have orientation training since they were household members.

Plan of Correction: The licensee will review the material for orientation and conduct this training with caregiver #2 and caregiver #3 within 2 weeks and document such training in their provider records.

Standard #: 8VAC20-800-210-A
Description: Based on a review of records and interview, the licensee did not ensure that all caregivers obtain a minimum of 16 hours of training annually in areas relevant to their job responsibilities.
Evidence:
Caregiver #1 and caregiver #2 were both observed caring for infants in the home at the time of the inspection on 11/17/2022.
The records for Caregiver #1 and caregiver #2 did not contain documentation of any annual training .
When asked for documentation of training for Caregiver #1 and caregiver #2, the licensee stated that she was not aware that Caregiver #1 and caregiver #2 had to obtain annual training hours.

Plan of Correction: The licensee stated that she will have both Caregiver #1 and caregiver #2 begin annual training now, with the goal of completing a minimum of 16 hours of applicable training within 12 months.

Standard #: 8VAC20-800-350-E
Description: Based on thermometer readings taken at the time of inspection, the licensee did not ensure that hot water taps available to children are maintained within a range of 105 to 120 degrees Fahrenheit.
Evidence:
The water temperature measured 122 degrees Fahrenheit with a digital thermometer at the time of the inspection on 11/17/2022.

Plan of Correction: The licensee has requested help in adjusting the temperature on her water heater and will move it to to below 120 degrees Fahrenheit, today, 11/18/2022.

Standard #: 8VAC20-800-520-A
Description: Based on observation and interview, the licensee did not provide cribs for enrolled children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed.
Evidence:
At the time of inspection, there were two infants under the age of 12 months old in care: Child #1, 4 months old and child #5, 8 months old.
No cribs were observed and the licensee confirmed that she did not have any cribs available for sleeping infants.
There was one mesh sided playpen which the provider stated that she used in addition to a basinet which was not observed.
Infants in care under the age of 12 months are required to sleep in an individually assigned crib.

Plan of Correction: The licensee acquired one crib on 11/18/2022 and has ordered a second crib which is being delivered. Infants older than 12 months of age will sleep on cots with appropriate supervision,.
