Technical Assistance:
Discussed with the provider ratios and capacity, background checks and TB screening requirements, subsidy requirements and immunization records.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 9:50am -11:20am. There were 10 children (30 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-2
Description: Based on record review, the Family Day Home did not obtain Fingerprint results for a staff member prior to their first day of employment.

Evidence:
Staff #2's record contains Fingerprint results that were completed on 6/9/2022. The staff member's first day of employments was documented as 6/1/2022.

Plan of Correction: Violation will be corrected by making sure to obtain all documents prior to the first day of employment.

Standard #: 8VAC20-770-60-B
Description: Based on record review, the Family Day Home, did not have a staff member complete a sworn statement or affirmation, prior to their first day of employment.

Evidence:
Staff #2's record contains a sworn disclosure or affirmation document, that was completed on 6/7/2022. The staff member's first day of employment was documented as 6/1/2022.

Plan of Correction: Violation will be corrected by making sure to obtain all documents needed prior to employment date.

Description: Based on record review, children's records did not all contain required information.

Evidence:
Child #4's record is missing 1 of 2 required emergency contacts.

Plan of Correction: Violation was corrected by adding an emergency contact the same day of inspection.

Description: Based on record review, the Family Day Home did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child #5's record contains an immunization documentation that was last updated on 1/8/2022.

Plan of Correction: Violation has been corrected by obtaining an updated immunization record.

Standard #: 8VAC20-800-680-B
Description: Based on observation, children's hands were not washed before eating.

Evidence:
The caregivers were observed placing three children in high chairs and giving each child a snack without washing their hands.

Plan of Correction: Violation is being corrected by making sure to follow rules and guidelines.
