Comments:
An unannounced renewal inspection was conducted on November 17, 2022. The inspector was on site from approximately 12:30 pm until 2:45 Pm. There were ten children present with two caregivers supervising for a value of 18 points. Two additional volunteers were also in attendance. Children were observed finishing lunch, playing and later napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records, two caregiver records, one household record and two volunteer records were reviewed. The inspector interacted with caregivers and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the licensing inspector.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review of ten children?s records, the provider did not obtain documentation of a physical for three children?s records prior to the child?s attendance or within 30 days after the first day of attendance.

Evidence:

1. The record of child #2 (enrolled 10/18/2022), child #5 (enrolled 07/21/2021) and child #9 (enrolled 10/10/2022) did not contain documentation of a physical exceeding 30 days from the date of attendance.
2. The provider stated she did not have documentation of a physical for the three children?s records.

Plan of Correction: Per the provider: I will call the parents now and request they send me the missing documentation for the physicals.

Standard #: 8VAC20-800-510-A
Description: Based on observation and interview, the provider did not ensure children were provided with an individual crib, cot, rest mat or bed for resting or napping.

Evidence:

1. Three children (Child #1, child #5 and child #9, all two years in age) were observed napping on the two sofas in the family room.
2. 2. The provider stated the three children wouldn?t sleep on cots so she allowed them to sleep on the sofa.

Plan of Correction: Per the provider: I will not have the children sleep on the sofas. I will purchase mats to see if the children will sleep on them better than the cots.

Standard #: 8VAC20-800-530-A
Description: Based on observation and interview, the provider did not ensure when cribs, cots, rest mats, and bed are used for sleeping or napping by children other than infants, linens shall consists of a top cover and a bottom cover or a one-piece coving that is open on three edges.

Evidence:

1. One child was napping on a cot without a top and bottom cover. A second child had a small blanket on top but no bottom cover.
2. Three children sleeping on the sofa did not have bottom covers.
3. The provider stated the children would not sleep on the cots so they were allowed to sleep on the sofa.

Plan of Correction: Per the provider: I will make sure the children have both a top and bottom cover during nap time.
