Comments:

A renewal inspection was conducted on 11/28/2022 between the hours of approximately 9:45 am and 12:00 pm. There were 11 children present (26 points), ranging in ages from 16 months to 4 years, with 2 staff supervising. Children and staff were observed while coloring, circle time and free toy play. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. According to the provider, there was no medication on site today. A total of 3 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-B-2
Description:	Based on record review, the provider did not ensure to obtain fingerprint-based criminal history check determination letter prior to first day of employment for one caregiver. Evidence: 1) On the date of inspection, the record for Caregiver #4 did not contain a current fingerprint-based criminal history check test result or documentation that a criminal history check had been requested.
Plan of Correction:	Will work on her paperwork.
Standard #:	22VAC40-191-150-B
Description:	Based on record review, it was determined that the provider did not ensure that Central Registry search results were obtained by the end of the 30th day of employment. Evidence: 1) On the date of inspection, the record for Caregiver #4 (start date 09/09/2022) did not contain the results of a Central Registry search request nor documentation that a Central Registry search had been requested.
Plan of Correction:	Will work on her paperwork.
Standard #:	8VAC20-800-100-A
Description:	Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1) The physical examination in the record for Child #3 (start date 06/08/2020) was dated 06/04/2021.
Plan of Correction:	Will ensure it doesn't occur again.

Description:	Based on record review, one caregiver's record did not contain all required information. Evidence: 1) The record for Caregiver #4 did not contain the date of employment or volunteering, and name, address and telephone number of a person to be notified in an emergency.

Description:	Based on record review, the provider did not obtain documentation that one child had been adequately immunized before the child attended the family day home. Evidence: 1) The immunization report in the record for Child #3 (start date 06/08/2020) was dated 06/04/2021.
Plan of Correction:	Will ensure child is fully vaccinated.

Description:	Based on record review, the provider did not orient one assistant by the end of their first week of assuming job responsibilities. Evidence: 1) The record for Caregiver #4 (start date 09/09/2022) did not contain documentation of having received orientation training from the provider.

Standard #:	8VAC20-800-690-D
Description:	Based on observation, the diapering surface did not meet all requirements. Evidence: 1) The inspector observed that the diapering mat had rips on it preventing it from being nonabsorbent and washable.
Plan of Correction:	Ordered a new one immediately.
