Violations:
Standard #: 8VAC20-800-50-A
Description: Based on interview, the family day home does not keep a written record of children in attendance each day.
Evidence: When asked for todays attendance, the provider stated that she hadn't completed attendance that day.

Plan of Correction: I always use the attendance form but I didn't use it today because it's a short week. I'll do it.

Standard #: 8VAC20-800-50-E
Description: Based on interview, records were not maintained and made accessible to the departments representative for two years.
Evidence: When asked for documentation of last weeks attendance, the provider stated that they do not keep their weekly attendance records.

Plan of Correction: I'll start keeping them.

Standard #: 8VAC20-800-60-B
Description: Based on record review children's records do not contain all required information.
Evidence: Child #3's record does not contain two emergency contacts, authorization for medical attention, documentation that the parents have been informed of the providers policies, emergency procedures or documentation of liability declaration.

Plan of Correction: I'll tell the mom that I need the forms returned today.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that each child has been adequately immunized before attending the family day home.
Evidence: Child #3's record does not contain documentation of immunizations.

Plan of Correction: I'll tell her today that I need the record in two weeks.

Standard #: 8VAC20-800-530-G
Description: Based on observation, children under two years of age were permitted to use pillows.
Evidence: Child #1 (18 months old) was observed in a crib with a pillow.

Plan of Correction: I only put the child in the crib because you are here. They usually sleep in a swing. Corrected during the inspection.
