Comments:
This initial monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person inspection was conducted of the program.
The inspection was initiated on December 5, 2022 and concluded December 6, 2022. There were no children present. The provider and 3 household members were present.
Inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 3 records were reviewed, 1 provider and 2 household member'.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-800-280
Description: Based on observation and provider interviews, the provider did not ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

Evidence: There were three separate areas where hazardous materials were determined accessible to children. These items were labeled "keep out of reach of children" and labeled with at least one other statement "warning", "flammable" and "hazardous".
1) On a shelf in the foyer area and on the cubby where children store their belongings there was a container of hand sanitizer.
2) In the restroom designated for children there was one room deodorizer.

Plan of Correction: All hazardous materials were removed in all three areas to an area that is inaccessible to children.

Standard #: 8VAC20-800-350-E
Description: Based on observation and provider interviews, the provider did not ensue that hot water at taps available to children shall be maintained within a range of 105?F to 120?F.

Evidence: The hand sink in the restroom designated for the enrolled children, had hot water that was more than the range required. The provider?s household member checked the hot water heater during inspection and confirmed it was set for 125 F degrees.

Plan of Correction: Household member checked hot water heater and set it to 110F.

Standard #: 8VAC20-800-500-C
Description: Based on observations and measurements taken, the provider did not ensure that stationary playground equipment was placed at least six feet from the perimeter of other play structure or obstacles.

Evidence: A composite piece / play structure was observed with a playground boarder and two slides attached and it did not have a minimum of 6 feet free from obstacles for fall zone as required.
1) The obstacle for the wavy slide chute was the playground boarder approximately 2 `10? from the slide chute.
2) The obstacles for the curled tube slide chute were a playground boarder approximately 2?10? from the slide chute and a tree that is approximately 4?10? from the slide chute.
3) The playground boarder was placed around the entire play structure and did not allow for a minimum of 6 feet around the permitter the play structure.

Plan of Correction: Provider has roped off slides to playground (12-5-22) and will not be used by children until playground structure is moved away from tree and the playground boarder is fixed to allow 6 feet from obstacles. in care until household member can remove trampoline (12-11-2022).

Standard #: 8VAC20-800-500-D
Description: Based on observations and measurements taken, the provider did not ensure outdoor play equipment shall meet all the requirements.

Evidence: The wooden piece of stationary equipment had one swing with "S" hooks used to attach the swing and it was not tightly closed (approximate 1? opening).

Plan of Correction: "S" hooks were adjusted to baby swing to be tightly closed.

Standard #: 8VAC20-800-500-F
Description: Based on observations and measurements taken, the provider did not ensure that trampolines would not be used for children in care.

Evidence: The blue metal piece of stationary equipment had an attached trampoline that measures 42? by 42? and is 7 ?? high.

Plan of Correction: Provider has roped off trampoline (12-5) to ensure that it is not used for children in care until household member can remove trampoline (12-11-2022).
