Technical Assistance:
Discussed the following:
*keep an eye on wooden picnic table on the outdoor play area - to ensure wood does not deteriorate further.
*Check equipment each morning for standing water.
*Two shelter-in-place practice drills are required annually.
*Definition of "Assistant" reviewed.

Comments:
An unannounced renewal inspection was conducted on 12/07/2022 from 9:30 am - 10:50 am. At the time of entrance there were four children in care with one caregiver. (11 points) At the time of departure there were six children in care with two caregivers. The ages of the children counted for a total of 18 points.
Children were observed engaged in painting and art in the child care room with both caregivers. Clean up and free play was implemented after art.
Records were reviewed for two caregivers and five children.

The information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-800-960-E
Description: Based on observation and interview, the provider failed to ensure that bottles shall be labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
Four bottles of infant formula, for two different infants in care, were not dated or labeled with the child's full name.

Plan of Correction: The provider stated:
All bottles will be dated and labeled with the child's name if more than one infant is in care.

Standard #: 8VAC20-800-170-B
Description: Based on record review, the provider failed to ensure that the TB screening form shall have been completed within the last 30 days of hire.

Evidence:
1. The TB screening for caregiver 1, hire date 11/19/2022, was completed on 03/21/2022.

Plan of Correction: The provider stated:
Going forward, all new caregivers will obtain a TB screening that is no older than 30 days prior to hire.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider failed to ensure that outside equipment of the family day home shall be maintained in a safe condition.

Evidence:
1. The wooden cartoon characters attached to the outdoor fence have areas of peeling paint that comes off on hands when touched.

Plan of Correction: The provider stated that the characters had been sanded down and repainted nevertheless, the weather has caused the paint to peel again. The characters will be repainted and a clear coat will be applied to preserve the paint and prevent peeling.

Standard #: 8VAC20-800-490
Description: Based on observation, the provider failed to ensure that climbing portions of indoor slides and climbing equipment over 18 inches high shall not be over bare floor.

Evidence:
1. A plastic play structure with slide was placed over the wood floor in the child care play room.
a. A child was observed playing on this slide.

Plan of Correction: The provider moved the slide onto the carpet during the inspection.
