Comments:
An unannounced renewal inspection was conducted today from 12:30pm-1:25pm. There were 6 children for a total of 20 points directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today and the Provider is not MAT certified. Children were observed napping various rooms. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-830-C
Description: Based on interview and record review, it was determined the provider did not maintain documentation of shelter-in-place drills.
Evidence:
1. Upon review of emergency response drills, only evacuation drills were recorded on the sheet for the last year.
2. The Provider stated she did the drills but did not record it on the sheet.

Plan of Correction: In the future, I will add it onto the emergency record.

Standard #: 8VAC20-800-100-A
Description: Based on record review, it was determined the provider did not obtain documentation of a physical examination for two enrolled children within 30 days after the first day of attendance.
Evidence:
1. Child #1's record and Child #2's record did not contain a physical report and both started in the Program in August, 2022.

Plan of Correction: I had a document in the file that I believed was sufficient for a physical but I will request one from them.

Standard #: 8VAC20-800-70-B
Description: Based on record review, it was determined the Provider could not demonstrate that she obtained all of the parent's written acknowledgement of the receipt of the following information.
Evidence:
1. 4 children's files were reviewed and 2 were missing parent written acknowledgement about the Provider's Emergency Preparedness Plan and that the Provider does not carry liability insurance.

Plan of Correction: I have given them forms but I will make sure to obtain documentation of the info from the parents.

Standard #: 8VAC20-800-90-A
Description: Based on record review it was determined the Provider did not obtain, before a child attended the family day home, documentation that the child has been adequately immunized.
Evidence:
1. Child #2 has been enrolled in the home since August, 2022 and does not have an immunization record.

Plan of Correction: I will request the information from the parent.
