Comments:
An unannounced monitoring inspection was conducted on December 13, 2022. The inspector was on site from approximately 11:30 am until 1:00 pm. There were six children present with two caregivers supervising for a value of 15 points. Children were observed napping. One child was awake and was being cared for by the assistant who was talking with her. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records, three caregiver records, and two household member records were reviewed. The inspector interacted with caregivers and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the licensing inspector.

Standard #: 8VAC20-800-800-B
Description: Based on interview and review of documentation, the provider did not review the emergency preparedness plan at least annually and document in writing each review and update to the emergency plan.

Evidence:

1. The provider stated that the emergency preparedness plan had been reviewed however the most resent date of review documented was 08/31/2020, exceeding the annual requirement.
2. The provider stated she had forgotten to document the date reviewed.

Plan of Correction: Per the provider, we will review the emergency preparedness plan today and document the review.

Standard #: 8VAC20-800-140-B-4
Description: Based on review of caregiver records, the provider did not obtain all required documentation for the substitute provider?s record.

Evidence:

1. The record for caregiver #3, identified as the substitute provider, did not contain documentation of a high school program completion or the equivalent or evidence of having met the requirements for admission to an accredited college or university; documentation of three months of programmatic experience; documentation of current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care; documentation of current certification in first aid.
2. The provider stated she did not have the required documentation for the substitute?s record.

Plan of Correction: Per the provider: I will sigh the caregiver up for the CPR and first aid class. I will also have her provide me with documentation of her education.
