Reviewed repeat violations from previous inspections 07/13/2022.

Comments:
An unannounced renewal inspection was conducted on December 14, 2022. There were 3 children present (6 points) with one caregiver supervising. The children were observed watching a video. The home, children's records, caregiver records, household member records, evacuation drills, attendance drills, first aid kit, first aid certifications and emergency preparedness procedures were reviewed. Violations:
Standard #: 8VAC20-800-760-A
Description: Based on observation, the provider failed to ensure that the first aid kit contained all required supplies.

Evidence:
The first aid kit was missing 2 triangular bandages.

Plan of Correction: I will purchase 2 triangular bandages for the first aid kit.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days of the first day of attendance.

Evidence:
Child #3 and #4's record do not contain documentation of a physical.
Child #3's first day day of attendance was 9/9/2022. Child #4's first day of attendance was 9/19/2022.

Plan of Correction: I will get the physicals from the parent and place in children's files.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documents for each child enrolled.

Evidence:
- Child #2 and #4's records do not contain documentation of the home address of each parent who has custody.
- Child #1, #2, #3 and #4's records do not contain documentation of the name and/or the address and/or the phone number for the place of employment of each parent who has custody.
- Child #1 and #2's records do not contain documentation of the addresses of the emergency contacts.
- Child #1's record does not contain documentation of the names of person other than custodial parents authorized to pick up the child.

Plan of Correction: I will contact the parent's to get the information and will place information in children's files.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children records, the provider did not obtain documentation that the child has been adequately immunized according to the requirement of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
Child #1 and #2's records do not contain documentation of immunizations on the first day of attendance. Child #1 first day of attendance was 9/9/2022 and immunizations are dated 11/22/2022. Child #2's first day of attendance was 9/19/2022 and immunizations are dated 11/01/2022.

Plan of Correction: New children will not be allowed to attend without immunization records.
