Comments:

An in-person mandated renewal inspection was conducted on December 13, 2022 from 10:00am until 10:45am. There were 10 children in care (24 points) with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing with playdough and slime. An infant was observed sleeping. A total of 3 child records, 2 staff records and 2 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review, all household members did not submit to fingerprinting. Evidence: Household member 1 did not submit to fingerprinting.

Description:	Based on record review, all household members did not complete all required background checks when they became 18 years old. Evidence: Household member 1 did not complete a sworn statement.
