Comments:

An unannounced renewal inspection was conducted today between 11:40am and 1:15pm. There were 5 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed playing freely, washing hands, eating lunch. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Description:	Based on record review and interview, the provider did not obtain a background check for each employee every five years. Evidence: Caregiver #2s central registry background check was dated 4/12/2017 and had expired on 4/12/2021. The expired central registry background check was not renewed until 11/18/2022.
