A monitoring inspection was initiated on 12/21/2022 and concluded on 12/21/2022. There were a total of 9 children with 3 staff members present. Children were observed earing bananas and individual play.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child, 2 staff and 2 household records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-100-A Description: Based on children records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2 did not a have a physical examination in the record prior to a attendance or within 30 days after the first day of attendance.Plan of Correction: I will let the parents know child's form need to be completed.
