A monitoring inspection was initiated on 12/20/2022 and concluded on 12/20/2022. There were 8 children present, ranging in ages from 11 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-800-240-A Description: Based on observation, outside equipment was not maintained in a safe and operable condition.
Evidence:
1. The wooden climbing structure on the playground had a split board.Plan of Correction: I will get my husband to get out there and fix that board so no children can get a splinter or get hurt.
