Comments:
An unannounced monitoring inspection was conducted with the provider beginning at 12:41 PM and ending at 1:23 PM. There were five children, ages 19 months, 2, 2, 4, and 4 years old, in the care of the provider and one assistant for a total of 11 points. The children were observed asleep on cots during nap time. Three children's records were reviewed. Two household member's records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent emergency escape drill was documented as 10/03/2022. The most recent shelter-in-place drill was documented as 10/03/2022.

Description: Based on a review of documentation, the provider did not ensure that an emergency evacuation drill was conducted on a monthly basis. Evidence: There was no emergency practice drill documented for November 2022.

Plan of Correction: The provider will ensure that an emergency evacuation drill is conducted and documented on a monthly basis.

Standard #: 8VAC20-800-60-B
Description: Based on a review of documentation, the provider did not ensure that all of the children's records contained the required information. Evidence: Child A and child B's records were missing the medical insurance name and policy number. Child C's record was missing the parent's work place information and one emergency contact.

Plan of Correction: The provider will requested the missing information from the parents and update the children's records.
