Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:45 am through 12:30 pm. There were a total of nine children (22 points) observed with three caregivers. A total of six children's files and five staff/household member's files were reviewed. The children were engaged in the following: toy play, handwashing, bathroom breaks, diapering, lunch and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the family day home did not ensure that all staff undergo a repeat Virginia Central Registry of Child Abuse and Neglect search every five years.
Evidence:
1. A Central Registry Search was not conducted after five years. Caregiver #1's record contained an expired Central Registry Finding. The Central Registry Finding was dated 11/20/2017.
2. A Central Registry Search was not conducted after five years. Caregiver #2's record contained an expired Central Registry Finding. The Central Registry Finding was dated 12/13/2017
3. A Central Registry Search was not conducted after five years. Caregiver #3's record contained an expired Central Registry Finding. The Central Registry Finding was dated 11/20/2017.
4. Caregiver #1 confirmed that a new Central Registry Search had not been conducted at the time of the inspection.

Plan of Correction: The provider will use the portal system to complete a Central Registry Check for all expired Central Registries. Once the findings are returned, the provider will email the results to the LI.

Standard #: 8VAC20-800-50-A
Description: Based on observation and interview, the family day home did not keep a written record of children in attendance each day.
Evidence:
1. Attendance for 01/03/23, 01/04/2023, and 01/05/2023 was not taken and documented.
2. Caregiver #1 confirmed that attendance was not documented for those days.

Plan of Correction: Attendance will be taken upon arrival of all children. The provider will complete attendance for 01/03, 01/04, and 01/05.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's did not contain required information.
Evidence:
1. Child #1's record (Start date-10/12/2022) did not contain documentation of the medical insurance policy number.
2. Child #6's record (Start date-11/28/2022) did not contain documentation of the medical insurance and policy number.

Plan of Correction: The provider will get the medical insurance and policy numbers from the parents and document the record.

Standard #: 8VAC20-800-80-B
Description: Based on record review, the provider did not verify the identity and age of a child.
Evidence:
1. Child #6's record (Start date-11/28/2022) did not contain documentation of proof of age and identity.
