Technical Assistance:
Discussed with the provider: license modification, menu, updating attendance records upon arrival and departure of children, using a table during meals for older children who cannot fit or cannot be strapped properly in high chairs

Comments:
An unannounced monitoring inspection was conducted on 01/05/2023 between the hours of approximately 11:35 a.m. and 12:50 p.m. There were 13 children (1 of 13 resides in the home) present with the Provider and 2 assistants for a total of 33 points within the supervision guidelines. The children were observed eating lunch and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, and attendance were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.058
Description: Based on observation and caregiver interview, the family day home did not have a working carbon monoxide detector.

Evidence: Caregiver #1 stated that there was not a carbon monoxide detector in the home.

Plan of Correction: I will buy one today.

Standard #: 8VAC20-800-340-C
Description: Based on observation, the bathroom did not contain paper towels.

Evidence:
1. The bathroom used by the children in care did not contain paper towels.

2. Child #2 was observed drying their hands with a cloth towel in the bathroom after washing their hands.

Plan of Correction: I will put some in the bathroom today.

Standard #: 8VAC20-800-540-A
Description: Based on observation, high chair safety straps were not used and securely fastened, when occupied by a child.

Evidence:
1. The high chair safety strap was not fastened as Child #2 sat in the chair for lunch.
2. The high chair safety strap was not fastened as Child #5 sat in the chair for lunch.
3. The high chair safety strap was not fastened as Child #6 sat in the chair for lunch.
4. The high chair safety strap was not securely fastened as Child #8 sat in the chair for lunch.
5. The high chair safety strap was not fastened as Child #2 sat in the chair for lunch.

Plan of Correction: We will use the straps.
