Comments:
An unannounced monitoring inspection was conducted on 01/09/2023. There were 3 children present with 1 caregiver. The home, cribs, attendance logs, evacuation logs, children's records, staff records and household member records, the first aid kit, battery operated radio and flashlight were reviewed. Violations:
Standard #: 8VAC20-800-830-A
Description: Based on a review of records and discussion with the provider, the provider failed to ensure that the emergency evacuation procedures shall be practiced monthly with all caregivers and children in care.

Evidence:
The provider did not have documentation of an emergency evacuation drill monthly. There was no drill for December 2022.

Plan of Correction: I will complete the fire drill practice each month.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #2's record does not contain documentation of a physical examination by the 30th day of attendance. First day of attendance 09.06.2022.

Plan of Correction: I will contact the parent and ask for a physical for the child.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2 and #3's records do not contain documentation of the telephone number of each custodial parent's place of employment.
Child #3's record does not contain documentation of the addresses of two designated persons to contact in case of an emergency if the parents cannot be reached.
Child # 2, #3 and #4's record do not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.
Child #3's record did not contain documentation of the proof of the child's age and identity,

Plan of Correction: I will ask parents for the information and will put it in each child's file.
