22.1 BACKGROUND CHECKS; CARBON MONOXIDE

Comments:
An unannounced renewal study was conducted from 12:00 pm - 1:00 pm. There were two children in care with the provider for a total of eight points. The children were observed resting, watching television, and eating. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, one provider record, and four household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-960-C
Description: Based on observation of the family day home it was determined that not all high chairs with safety straps were utilized while occupied. Evidence: The LI (licensing inspector) observed that while Child A (Age: 13 Months) was seated in a high chair the safety straps were not fastened around the child.

Plan of Correction: I will utilize the high chair safety straps

Standard #: 8VAC20-800-100-A
Description: Based on review of two children's records it was determined that not all records contained documentation of a physical within the first 30 days of attendance. Evidence: The record for child A (start date: 08/01/22) and Child B (start date: 10/14/22) did not have documentation of a physical on file.

Plan of Correction: I have reached out to the parents to provide correct documents.

Standard #: 8VAC20-800-80-A
Description: Based on review of two children's records it was determined that not all records contained documentation of proof of identity within the first seven days of attendance. Evidence: The record for Child B (start date: 10/14/22) did not have documentation of proof of identity on file.

Plan of Correction: I have reached out to the parents to provide correct documents.

Description: Based on review of four household member records it was determined that not all records contained documentation of a TB (tuberculosis) screening prior to coming into contact with children. Evidence: The record for Household Member A who moved into the home 12/16/22 did not have documentation of a TB screening on file.

Plan of Correction: Household member will take a TB test.

Description: Based on observation of the family day home it was determined that not all potentially hazardous substances were inaccessible to children. Evidence: The following hazardous substances were observed in the bathroom of the family day home and were accessible to children: 1. Hand sanitizer and mouth wash on the bathroom sink. 2. Air freshener under the unlocked cabinet under the sink. 3. Shampoo, soap and lotion were located in the tub/shower.

Plan of Correction: I will remove all hazardous items and fix the unlocked cabinet

Standard #: 8VAC20-800-530-F
Description: Based on observation of the family day home it was determined that items such as pillows, comforters, and stuffed toys were permitted in cribs with infants. Evidence: The LI (Licensing Inspector) observed the following prohibited items in a crib occupied by Child B (Age: 12 Months) who was sleeping. 1.) A full sized pillow positioned to the right side of the child. 2.) A soft comforter covering Child B from the neck down. 3.) Three plush toys were observed at the foot of the crib.

Plan of Correction: I will remove all items from crib.
