Comments:
An unannounced monitoring inspection was conducted today between 12:50pm and 1:45pm. There were 4 children (14 points) present with the Provider within the supervision guidelines. The children were observed taking a nap and playing with toys. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, caregiver, and household member records, evacuation drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Violations:
Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, 1 out of 4 children's records reviewed did not contain the required information.

Evidence:
1. Child #2 (doe: 8/22/2022) was missing the name, phone number, and address of 1 out of 2 required emergency contacts.

Plan of Correction: I will ask the parents for the emergency contact information today.

Standard #: 8VAC20-800-180-A
Description: Based on a review of household member records, 1 out of 3 adult household members did not have a current Report of Tuberculosis Screening for two years from the date of the first screening.

Evidence:
1. Household member #2's most recent Report of Tuberculosis Screening on file at the time of the inspection was dated 6/13/2020.

Plan of Correction: The household member will get the TB test results as soon as possible.

Standard #: 8VAC20-800-480-D
Description: Based on observation, all equipment used by children was not used in accordance with the manufacturer's instructions.

1. Child #2 (11 mos.) was observed sleeping in the car seat. The car seat was not being used in accordance with the manufacturer's instructions.

Plan of Correction: The child was immediately removed from the car seat and moving forward I will not allow children to sleep in car seats.

Standard #: 8VAC20-800-530-F
Description: Based on observation, soft bedding was used around infants.

Evidence:
1. Child #2 (11 mos.) was observed sleeping in a car seat with fuzzy blanket around the infant.

Plan of Correction: The blanket was immediately removed and I will not use blankets around the infants moving forward.
