Comments:

An unannounced renewal inspection was conducted on January 12, 2023 from approximately 1:45 pm to 2:45 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record and 5 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 5 children in attendance for a total of 16 points. The children were observed taking a nap while the inspector was on the premises. The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted. Evacuation drills were conducted monthly with the most recent on 01/05/2023. The information gathered during the inspection determined violations with applicable standards or law. Violations were issued. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 01/18/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #:	8VAC20-800-100-A
Description:	Based on records review, the provider failed to obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. Evidence: The record for child #3, date of enrollment 10/18/21, did not contain documentation of a physical examination.
Plan of Correction:	Per the provider "parent requested a copy of child physical form from Physician."
Standard #:	8VAC20-800-510-C
Description:	Based on observation, the provider failed to ensure that there is at least 12 inches of space in between each crib. Evidence: There were 3 cribs along the back wall that were all touching each other with no space in between. Each crib was occupied by a sleeping child.
Plan of Correction:	Per the provider "the cribs are on the wall due to the small space but I have put dividers in between each crib."
