An unannounced non-mandated monitoring inspection was conducted on 01/18/2023. There were 12 children, ages 5 months-3 years, totaling 29 points, and a total of 3 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, physical equipment and environment, care of children, preventing the spread of disease, emergencies and nutrition. A total of five children?s records and one caregiver record were reviewed. The children were observed during free choice play and nap time. The inspector suggested that the provider attend the upcoming Family Day Home Phase Training that is scheduled for February 14, 2023. The inspector arrived for the inspection at 2:00 pm and departed at 3:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-100-A Description: REPEAT VIOLATION
Based on record review and interviews, the provider failed to ensure that prior to a child?s attendance or within 30 days after the first day of attendance, documentation of a physical examination by or under the direction of a physician shall be obtained.
Evidence:
1. The record for Child 1 contained a documented start date of 10/10/2022; the record did not contain documentation of a physical examination.
2. Caregiver 1 stated that the start date for Child 3 was 11/29/2021; the record did not contain documentation of a physical examination.Plan of Correction: The parents were notified during the inspection of the missing information.
Standard #: 8VAC20-800-60-B Description: REPEAT VIOLATION
Based on record review, the provider failed to ensure that children?s records shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the following information: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there were no addresses provided for the two designated contacts).
2. The record for Child 4 did not contain the following information: the first date of attendance.
3. The record for Child 5 did not contain the following information: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there were no addresses provided for the two designated contacts); and information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.Plan of Correction: The parents were notified during the inspection of the missing information.
