Each child's record will need the child's record form, Information for Parents form, decision to administer medication form, liability declaration form, provision of emergency relocation form, health and immunization records and signed proof of Identity.

Comments:
An announced initial study was conducted today. Three children were in care as the provider is currently permitted by Office For Children. The point total was eight with two caregivers which is within ratios for supervision. Observed the entire home and outside play area, emergency preparation, diapering area, records for children, supplies and equipment including a crib and nutrition. Currently, the children bring lunch and food from home. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit is missing items.
Evidence:
No tweezers, a second triangular bandage, gauze pads, adhesive tape and the first aid manual are missing from the first aid kit.

Plan of Correction: Missing items wil be purchased and added to the first aid kit.

Standard #: 8VAC20-800-770
Description: Based on observation, a battery operated weather band radio was not available.
Evidence:
The batteries in the weather band radio were not working.

Plan of Correction: Batteries will be replaced.

Standard #: 8VAC20-800-790
Description: Based on observation, no posted emergency numbers are close to the telephone.
Evidence:
Poison Control and 911 numbers are not posted by the phone.

Plan of Correction: Numbers will be posted by the phone within a week.

Description: Based on review of the written emergency plan, it is missing some components for evacuation, shelter-in place and lockdown procedures.
Evidence:
The provider was using the written emergency plan she had when she was permitted which does not cover all required procedures.

Plan of Correction: The written emergency plan will be re-written to cover all required procedures as outlined in the model form.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child resistant protective covers were not on all unused electrical outlets.
Evidence:
Two outlet covers were missing in the large playroom. One by the small basketball hoop and one by the large mounted TV.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-800-330-A
Description: Based on statement of household member (HHM), no landline telephone is available.
Evidence:
The HHM stated that no landline was available and only cordless phone that requires electricity was available at this time.

Plan of Correction: The household member will reach out to the phone company and get a landline installed.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering pad is covered with a soft material cover and unable to be cleaned and sanitized.
Evidence:
The diaper pad is not a non-absorbent surface as it is covered with a material top.

Plan of Correction: The cover will be removed so the pad is non-absorbent and can be cleaned and sanitized after each use.
